Registration Form

The MFHS is currently taking online registrations for future vaccine clinics. Vaccines will be available to the MFHS patients, registered members of the Aroostook Band of Micmacs (ABM), employees of the ABM, and for immediate household non-native members of registered ABM as quantity allows. Please know that appointments will be prioritized in accordance with the MFHS COVID-19 vaccination priority plan approved by Tribal Council.
Please complete a registrations form for each person you wish to register. Currently accepting registrations for ages 16 years and older. Current vaccines are a two-shot series.

NOTE: If you have already received your first shot of a COVID-19 vaccine please do not fill out this form. This is for new clients only.
